I'd like to see the penalty begin immediately when a player steps on the ice for a fatigue shift, not on the second and subsequent shifts.
However, I'd like to see players allowed to play when fatigued freely with the current penalties (Same as Out of Position in 14.2, see below). So in short, we can freely play our players fatigued, but they will incur regular matrix penalties for Out of Position and incur Defense and Clearing penalties immediately, not after a shift. You will not need to wait to play a player fatigued for all players rated at that position to be fatigued.

3.3 Effects of Fatigue
Skaters may not play when fatigued. A fatigued skater must leave the ice immediately.
Exception: If all other skaters on the team that can play the skater’s current position are also
fatigued, the skater may continue to play.
A fatigued skater suffers all the same penalties as playing Out of Position (Rule 14.2). In addition, at the start of
the second and subsequent extra shifts played, there is a cumulative -1 penalty to the skater’s Defense and
Clearing ratings. Treat results below zero as zero.
A skater who is both Fatigued and Out of Position suffers the Out of Position penalties twice.
14.2.1 Effects on Action Code Results
Out of Position skaters suffer the following Action Code result penalties:
-1 to all Attempt Shot (N) results
-1 to all Automatic Shot (●) results. Treat the Automatic Shot as a 13 and apply penalty.
-1 to all Pass Attempt (Pn) results.
All Rush Attempt (Dn) results are intercepted and cannot be converted to pass
All Stretch Pass (@) results are intercepted and cannot be converted to pass
For any successful shot or pass result, reroll the Red Die. On a six, the puck is intercepted. Roll on the Pass
column of the skater until an opposing skater has Possession of the puck. This interception cannot be
converted to a pass, nor prevented in any way.
14.2.2 Effects on Ratings
Modify the skaters Defense with all the following that apply:

-1 Defense for playing out of position
-1 Defense for skaters rated forward playing in the LD or RD position
-1 Defense for skaters rated defense playing in the LW, C, or RW position.
Adjusted Defense lower than one is treated as zero.
A skater rated defense playing LW, C, or RW who has an Intimidation rating adds a Screener rating
